When I was responsible CRA for Opioid analgesic-Induced-Constipation study, my responsible site was selected for study audit. There was no major finding, but there are several minor findings. Some of findings were deficiencies in source documentation. For example, the site used electronic blood pressure gauge but the study coordinator didn’t collect the first source data from the machine. Since the study was about to close, it was not required to use blood pressure gauge anymore. CRA educated site staffs the meaning of source data and let them collect source data from next study. And another example is that the record of “no adverse event in this visit” was not documented at the patient chart even though Case Report Form checked in no adverse event. CRA was educated of good clinical practice and good documentation practice, and then I educated site staffs accordingly. Other finding was deficiency in version control of study documentation.
