Nobody wants to make a mistake, but we are human and we have to admit that as a human, we can make mistakes. We just need to not to repeat same mistake by admitting our faults. That\'s why many companies implement the reporting system through online or paper.
This is not for discipline ones who make a mistakes, this is for making a new system not to repeat a same things. Even though, the reporting system has a lot of advantages, people don\'t want to raise the issues because they might be treated worse. I don\'t think management team can understand any mistakes their employees make, especially if they make a critical mistakes. I think that the most important components to minimize the mistakes made by their staffs is to appoint not to discipline and deal it as a system problem.
I guess that all mistakes occur from the wrong system in the company. It is not a personnel problem.
